What is included in my persona healthcare direct membership?

Persona Healthcare Direct, through its community of Affiliated Providers, will deliver primary care services to each employee, their spouse and/or dependents that are currently enrolled in the Persona Healthcare Direct Primary Care Program.  These primary care services may include, but are not limited to:

  • Treatment of sprains and minor lacerations (service varies be provider)

  • Joint Injections (service varies by provider)

  • Weight management and health risk assessment

  • Women’s health

  • Well child exams (service varies by provider)

  • Preventative health care

  • Same-day or next-day appointments based on medical needs

  • Acute care and chronic disease management

  • Allergy evaluation

  • Dermatology evaluation (procedures excluded as outlined below)

  • Cryotherapy (service varies by provider)

  • Mental health, including treatment for depression, anxiety and other mood disorders

  • School, sports and annual wellness physicals

  • Office testing such as EKG, urinalysis. strep test and pregnancy test

  • Sleep assessments and support

  • Stress management

In addition, Persona has agreed to provide the following for its members:

  • Virtual doctor visits

  • Direct Access to Member’s Physician/Provider (via telemedicine, email and/or patient portal) 

  • Priority scheduling

  • 24/7 access to your physician

  • Coordination of urgent care, emergency and specialty care services

  • Wellness coaching

  • Smoking cessation and weight loss programs

The following dermatology procedures are excluded from the member services agreement and are provided at a cost for supplies (service varies by provider):

  • Shave biopsies or shave excision

  • Excisional biopsies, cyst excision, toenail removal and stitches
